We do require a $100 nonrefundable booking fee that must be paid when booking your appointment. (if applicable)
If a nonrefundable fee is collected it is credited towards the total cost of the appointment if and when you show up to your appointment and service is completed.
Upon booking you will receive 2 documents via email to initial/sign/and fill out. If this is not completed within 48 hours we have the right to cancel your appointment. (The docs you will receive is our prescreen form and a credit card authorization form - the credit card form will only be used if these policies are not upheld)
We require a card to be kept on file to book any appointment. We will not charge your card unless there is a violation to our Cancellation/Rescheduling Policy. Please familiarize yourself with that policy. Please note there is a 4% processing fee when a credit/debit card is used. We will take this 4% fee off day of your appointment if you choose to pay with cash.
Cards may be used to pay for your procedure if you shall choose. However, there is a 4% card fee on said transaction.
